SurveySez Initial Schema: For this assignment you will create a prospective version of an initial database design for the SurveySez project.
This design must include all tables and fields, along with the data types for each field. The design itself should take the form of a single SQL script, which should run properly at a MySQL command line.

So what is ACTUALLY due for this assignment?:
- A WORKING MySQL script to create prototype SurveySez database tables
- All fields must have appropriate data types
- Place this in a document or web page linked to your construction/homework page (image, word doc, text, no .vsd files, or other odd documents)
Can't Be Late: This is one of the few assignments that must be turned in on the due date!
Extra Credit: 10 extra points for any student who brings a printed copy to class on Tuesday!
(update: I've turned this assignment back on for a limited time. You can now turn this assignment in late but it will incur a potential 20% penalty)
